Subject: Important Notice Regarding Your Health Coverage Information (Form 1095-B)
In compliance with the Affordable Care Act (ACA), Associated Management Services is providing you with information about your health coverage for the tax year 2025.
Generally, you no longer need to file Form 1095-B with your federal taxes because it’s informational, proving you had coverage, and the penalty for being uninsured is $0 at the federal level. Most tax accountants no longer want these forms.
How to Access Your Form 1095-B:
You can request a copy of your Form 1095-B by contacting us at:

Important Information:
- AMS will provide your Form 1095-B within 30 days of receiving your request.
- This notice will remain on our portal through October 15, 2026.
- You do not need to file this form with your federal tax return, but you should keep it with your tax records as proof of health coverage.
